Facial Masculinization Surgery in Thailand

Get world-class care at vetted and accredited hospitals in Thailand.
Save up to 80% with internationally trained surgeons and state-of-the-art facilities.

Thailand has become a leading destination for Facial Masculinization Surgery, with JCI-accredited hospitals offering comprehensive FMS packages at costs 60-75% lower than Western countries. Surgeons at specialized gender-affirming centers perform 150+ FMS procedures annually using custom implants, virtual surgical planning, and craniofacial techniques, and trans men from the US, UK, and Australia choose Bangkok for this combination of surgical volume, specialized expertise, and immediate scheduling.

Facial Masculinization Surgery costs $8,000-$20,000 in Thailand for comprehensive packages (jaw augmentation, chin contouring, forehead procedures, rhinoplasty, Adam's apple enhancement) compared to $20,000-$50,000 in the US for surgery fees alone. Surgery dates available within 3-4 weeks at hospitals with dedicated FTM surgical teams, without the months-long wait times common in Western gender-affirming care.

Why Thailand for Facial Masculinization Surgery

  1. Specialized FMS surgical volume and expertise. Thai gender-affirming surgeons perform 150-200 FMS procedures annually at hospitals like Kamol Cosmetic Hospital. Dr. Kamol Pansritum has completed 10,000+ FFS and FMS procedures since 1997, with teams including plastic surgeons, maxillofacial surgeons, and craniofacial specialists. This surgical volume means refined techniques for jaw augmentation, brow ridge enhancement, and Adam's apple creation—procedures requiring deep understanding of both masculine and feminine facial anatomy.
  2. Cost savings of $12,000-$30,000. Complete FMS in Thailand costs $8,000-$20,000 all-inclusive (multiple procedures + CT planning + anesthesia + hospital stay + implants + follow-ups). The US charges $20,000-$50,000 for surgery fees alone, then adds $3,000-$5,000 for anesthesia, facility fees, and imaging. UK private clinics cost £15,000-£35,000. Australia $25,000-$45,000 AUD. Your savings cover flights, 3-week accommodation, and complete recovery expenses.
  3. Virtual surgical planning and custom implants. Thailand hospitals use 3D CT imaging and computer simulation to design patient-specific jaw implants, chin augmentations, and brow ridge prosthetics. Virtual planning allows surgeons to show predicted results before surgery, design wrap-around jaw implants for precise masculinization, and create surgical guides reducing operating time. Custom implants (Medpor, Porex, silicone) shaped specifically for your facial structure provide natural-looking, permanent results.
  4. Craniofacial surgery expertise. FMS requires more than standard plastic surgery—successful jaw and chin augmentation demands maxillofacial training for bone work and skeletal modifications. Thai FMS surgeons combine plastic surgery with craniofacial expertise, enabling them to perform complex procedures like forehead lengthening (moving hairline back), brow ridge implantation (creating prominent supraorbital ridge), and jaw angle augmentation (widening lower face). This specialized training differentiates quality FMS from basic implant placement.
  5. Immediate scheduling at JCI-accredited facilities. Book consultation today, receive virtual surgical plan within 1 week, fly to Bangkok in 3-4 weeks for surgery. Hospitals like Kamol Cosmetic Hospital maintain Joint Commission International accreditation—same standards as top Western facilities. No insurance battles, no 6-12 month surgeon wait lists. Comprehensive packages include pre-op CT scans, surgical planning, surgery, recovery monitoring, and revision planning if needed.

Understanding Facial Masculinization Surgery

What FMS Accomplishes

Facial Masculinization Surgery enhances traditionally masculine facial features that hormone therapy alone cannot change. Testosterone produces significant changes (facial hair growth, skin texture, fat redistribution) but does not alter bone structure. FMS addresses skeletal features defining facial masculinity:

Upper face (forehead and brow):

  • Higher, flatter forehead vs softer, rounded feminine forehead
  • Prominent brow ridge (supraorbital ridge) vs smooth brow transition
  • Greater distance between eyebrows and hairline
  • Lower, straighter eyebrows vs higher, arched feminine brows
  • Square hairline corners vs rounded feminine hairline

Mid-face (nose, cheeks, eyes):

  • Larger, wider nose with straight profile vs smaller, upturned feminine nose
  • Broader nasal bridge and more defined tip
  • Flatter, more angular cheekbones vs fuller, rounder cheeks
  • Slightly hollowed midface creating shadow/definition

Lower face (jaw, chin, neck):

  • Square, angular jaw with prominent jaw angle vs tapered, softer jawline
  • Broader mandible creating wider lower face
  • Longer, more projected, square chin vs smaller, rounded chin
  • Prominent Adam's apple (thyroid cartilage) vs smooth throat contour
  • Defined separation between jaw and neck

FMS surgically modifies these features through bone work, implants, and soft tissue procedures.

Common FMS Procedures

Forehead lengthening and augmentation: Increases distance between eyebrows and hairline. Surgeon removes strip of hair-bearing scalp, pulls hairline backward, creating higher forehead. Can combine with brow ridge implantation. Requires careful planning around existing hairstyle—receding hairline, short hair, or bald patients need different approaches to hide scarring.

Brow ridge augmentation: Creates prominent supraorbital ridge above eyes. Surgeon places custom implant or uses bone grafts to build out brow bone. Incision typically at hairline or within scalp. Most masculinizing single procedure—prominent brow ridge immediately reads as masculine even when other features remain unchanged.

Rhinoplasty (nose masculinization):Enlarges and straightens nose. Surgeon uses cartilage grafts (usually from ribs) to build up nasal bridge, widen nasal base, and increase projection. Creates broader, more angular nose harmonizing with masculinized jaw and forehead. Can address breathing issues simultaneously.

Cheek augmentation or reduction: Creates flatter, more angular midface. Depending on existing cheek structure, surgeon either places implants to create angular planes or reduces overly prominent cheekbones. Goal: eliminate rounded, full cheeks in favor of defined, shadowed cheekbone structure.

Jaw augmentation and contouring: Widens lower face and creates square jaw angle. Surgeon places wrap-around jaw implants through incisions inside mouth (no visible scars). Implants extend along jaw angle and sometimes continue forward to augment chin simultaneously. Increases face width, creates masculine jaw definition, establishes clear jaw-neck separation.

Chin augmentation (genioplasty): Lengthens and projects chin. Surgeon either slides chin bone forward (sliding genioplasty—permanent, uses your bone) or places custom implant. Can make chin wider, longer, more square. Often combined with jaw augmentation for total lower face masculinization.

Adam's apple enhancement: Creates visible thyroid cartilage prominence in throat. Surgeon makes small incision under chin or in neck crease, places Y-shaped cartilage implant (harvested from ribs) or silicone implant over existing thyroid cartilage. Neck muscles closed over implant protecting vocal cords. Expect temporary vocal weakness first 2-3 weeks.

Hairline masculinization: Raises and squares hairline. Combines hair removal (laser) with possible forehead lengthening. Creates M-shaped recession at temples characteristic of male hairlines. Some patients combine with scalp advancement if starting hairline is very low.

Your 3-4 Week FMS Journey in Thailand

Pre-Arrival (4-6 Weeks Before)

Virtual consultation through Konkai or direct hospital contact. Submit front, side, and 3/4 angle facial photos. Describe goals (which features feel dysphoria-inducing, which masculine traits you want enhanced). Surgeon provides preliminary assessment of candidacy and recommended procedures.

Requirements:

  • Age 18+ (18-20 requires parental consent)
  • Living as male for minimum 1 year
  • Hormone therapy for 1+ year (if applicable—some cisgender men seek FMS too)
  • Letter from mental health professional confirming gender dysphoria (for trans patients)
  • Good general health (controlled conditions acceptable)
  • Non-smoker or willing to quit 4 weeks before/after
  • Realistic expectations about surgical outcomes

Pre-surgery preparation:

  • Discontinue blood thinners, aspirin, NSAIDs, vitamin E (2 weeks before)
  • Stop smoking and alcohol (4 weeks before minimum)
  • Arrange 3-4 weeks off work (recovery time varies by procedures)
  • Pack comfortable clothing (avoid tight necklines if having Adam's apple work)
  • Bring copies of hormone therapy records and mental health letters

Week 1: Consultation and Planning

Day 1-2: Arrive in Bangkok, adjust to time zone and climate. In-person consultation with surgeon. Physical examination of facial structure, bone measurements, soft tissue assessment. Discuss specific goals for each facial region.

Day 3: CT scan of facial skeleton. Creates detailed 3D model of your bone structure for surgical planning.

Day 4-5: Virtual surgical planning session. Surgeon shows computer-generated predictions of post-op appearance. You review and request any adjustments to planned procedures. Finalize which procedures will be performed. CT images used to design custom implants if needed.

Day 6: Pre-admission testing. Blood work, EKG, chest X-ray (if over 40), HIV test (standard for surgery in Thailand). Final medical clearance.

Day 7: Free day. Implant fabrication begins (if using custom implants). Final preparation for surgery.

Week 2: Surgery Week

Surgery Day (typically Day 8-10):

What happens:

  • General anesthesia administered
  • Procedures performed in order determined by surgeon (typically upper face to lower)
  • If doing jaw/chin: incisions inside mouth, implants placed, secured with screws
  • If doing forehead/brow: incision at hairline or in scalp, bone work or implant placement
  • If doing nose: cartilage harvested from ribs if needed, nose reshaped and augmented
  • If doing Adam's apple: small neck incision, cartilage or implant placed over thyroid cartilage
  • Total time: 4-7 hours depending on number of procedures

Post-op (First 24-48 hours): Recovery room 2-4 hours monitoring breathing, swelling, vitals. Then private hospital room. Expect significant facial swelling (peak day 3-4). Pain managed with IV medications first 24 hours, then oral. Cannot eat solid food if had jaw/chin work—liquid diet first few days. Drains may be placed (removed day 2-3 depending on fluid output).

Day 2-4 (Hospital Stay): Continued monitoring. Swelling significant—you won't look like final result. Ice packs reduce swelling. Surgeon checks daily, assesses healing, removes drains when output drops. Liquid to soft food diet if jaw/chin augmented. Antibiotics prevent infection. Discharge once pain controlled with oral meds, swelling stable, eating/drinking adequately.

Week 3: Early Recovery

Hotel near hospital or in-hospital recovery apartment. Most challenging week—swelling peaks, movement restricted, soft food diet continues.

Swelling & bruising: Massive swelling day 3-5 (expect to look very puffy and not like yourself). Bruising extensive if had multiple procedures. Swelling gradually improves week by week but takes 6-12 months to fully resolve. You'll see approximately 60-70% of final result by month 3.

Activity restrictions:

  • Keep head elevated 30 degrees when lying down (reduces swelling)
  • No strenuous activity, heavy lifting, or exercise (4-6 weeks minimum)
  • Gentle walking encouraged (improves circulation)
  • Avoid bending over, straining, or anything raising blood pressure
  • If had Adam's apple work: vocal rest first week, gentle voice use week 2-3, expect hoarseness/weakness

Wound care:

  • If incisions in mouth (jaw/chin): rinse with saltwater after eating, soft food only
  • If scalp incisions (forehead): gentle hair washing after 5-7 days
  • If neck incision (Adam's apple): keep clean and dry, apply antibiotic ointment
  • Stitches mostly dissolve internally; external sutures removed day 7-10

Diet:

  • Jaw/chin patients: liquid diet days 1-5, pureed foods days 6-10, soft foods week 2-3, gradually return to normal
  • Other patients: eat normally but choose soft foods for comfort

Follow-up appointments: Every 2-3 days. Surgeon checks healing, removes sutures, drains fluid if accumulating, addresses concerns, photographs progress.

Pain level: 5-7/10 first few days managed with medications. Discomfort from swelling usually worse than surgical pain. Tightness, pressure, numbness common. If had rib cartilage harvested (for nose or Adam's apple): chest pain 4-6/10 for 1-2 weeks.

Week 4: Continued Recovery

Flying home: Minimum 14-21 days post-op depending on procedures. Surgeon provides clearance. Swelling still significant but improving. Wear loose clothing, stay hydrated during flight, walk aisles every hour.

Final appointment: Week 3-4. Surgeon removes any remaining external sutures, assesses healing, photographs results, provides long-term care instructions, discusses revision timeline if adjustments wanted (minimum 12 months wait for revisions).

Activity restrictions lifting:

  • Light desk work: Week 3-4
  • Driving: Week 3-4 (when can turn head comfortably)
  • Cardio exercise: Week 6-8 (start light, increase gradually)
  • Contact sports, heavy weights: Month 3-4 (after bone fully healed)

Numbness: Expect numbness in surgical areas. If had jaw/chin: lower lip and chin numbness 3-6 months (gradually returns). If had forehead: scalp numbness 6-12 months. If had nose: tip numbness 3-6 months. This is normal nerve recovery.

Long-term results:

  • Months 1-3: Swelling decreases significantly, can see emerging masculine features, numbness starts improving
  • Months 3-6: 80-90% of swelling resolved, final contours becoming clear, feeling returns to most areas
  • Months 6-12: Final shape settles as bone remodels around implants, remaining swelling resolves, scars fade
  • Permanent results: Implants permanent unless removed, bone work permanent, masculine facial structure lasts lifetime

Maintaining results:

  • Continue hormone therapy as prescribed (maintains masculine skin texture, facial hair, fat distribution)
  • Weight stability helpful (significant gain/loss changes facial fat, affecting appearance)
  • Skincare and sun protection (prevents premature aging, maintains surgical results)
  • No smoking (impairs healing, ages skin prematurely)

Procedure-Specific Details

Jaw and Chin Augmentation

Most commonly requested FMS procedures. Create square, angular lower face defining masculine appearance.

Jaw augmentation specifics:

  • Wrap-around silicone or Medpor implants along jaw angle
  • Increases width of lower face by 8-15mm each side
  • Creates prominent jaw angle visible in profile
  • Incisions inside mouth along lower gumline (no visible scars)
  • Implants secured with titanium screws to prevent migration
  • Results immediate though hidden by swelling

Chin augmentation options:

  1. Sliding genioplasty: Cut chin bone, slide forward and secure with plates/screws. Uses your own bone (permanent, natural). Can simultaneously lengthen, widen, and project chin. More invasive but most customizable.
  2. Chin implant: Silicone or Medpor implant placed over existing chin bone. Less invasive, quicker procedure. Good for moderate chin enhancement. Can be removed/replaced if desired.
  3. Combined approach: Sliding genioplasty for lengthening/widening + implant for additional projection.

Recovery specifics:

  • Liquid diet 5-7 days, soft foods 2-3 weeks
  • Lower lip numbness 3-6 months (temporary)
  • Swelling concentrated in lower face, obscures results first 6 weeks
  • Cannot fully open mouth first 2 weeks
  • Oral hygiene critical (saltwater rinses prevent infection)

Forehead and Brow Ridge Work

Highly masculinizing but less commonly requested than jaw/chin. Creates prominent upper face characteristic of male skulls.

Brow ridge augmentation details:

  • Custom implant or bone cement shaped to create prominent supraorbital ridge
  • Incision at hairline (1-2 inch scar hidden in hair) or within scalp (further back, better scar hiding but limits hairline adjustment)
  • Implant placed directly on frontal bone above eye sockets
  • Creates shadow over eyes, increases depth of eye sockets
  • Most dramatic single feature for facial masculinization

Forehead lengthening details:

  • Strip of scalp (1-2cm) removed along hairline
  • Hairline pulled backward, creating higher forehead
  • Sutures along new hairline position
  • Increases forehead height by 1-2cm
  • Creates more masculine forehead proportions
  • Critical consideration: Only works for patients with hair. Bald patients cannot do this procedure. Receding hairline patients need surgeon experienced in scar placement.

Recovery specifics:

  • Forehead swelling significant first 2 weeks
  • Upper eyelid swelling common (eyes may swell shut first few days)
  • Scalp numbness 6-12 months
  • Hairline scar fades over 12-18 months (use silicone scar gel)
  • Can wash hair gently after 7 days

Rhinoplasty (Nose Masculinization)

Enlarges and straightens nose to harmonize with masculinized jaw and brow.

Technique:

  • Rib cartilage harvested (typically 6-8cm from floating ribs)
  • Cartilage carved to create dorsal augmentation graft (builds up bridge)
  • Additional grafts widen nasal base, increase tip projection
  • Creates broader, straighter, more projected nose
  • Can address breathing issues simultaneously (deviated septum correction)

Results:

  • Masculine nose proportionate to other facial features
  • Wider nasal bridge (2-4mm increase)
  • Straighter profile (eliminates feminine upward slope)
  • More defined, projected tip

Recovery specifics:

  • External cast 7-10 days
  • Internal splints removed day 5-7
  • Chest pain from rib harvest (4-6/10) first 1-2 weeks
  • Nasal swelling takes 12+ months to fully resolve
  • Can breathe through nose after splint removal
  • Final shape visible month 12-18

Adam's Apple Enhancement

Creates visible thyroid cartilage prominence. Uniquely masculine feature.

Surgical options:

  1. Rib cartilage implant: Harvest cartilage from ribs, carve into Y-shape mimicking natural Adam's apple, place over existing thyroid cartilage. Most natural-looking, permanent, uses your tissue.
  2. Silicone implant: Pre-shaped silicone implant. Less invasive (no rib harvest), quicker procedure. Risk of migration or visibility over time.

Incision location:

  • Under chin (better scar concealment)
  • Neck crease (more direct access, visible scar but fades well)

Vocal considerations:

  • Procedure does NOT alter vocal cords (voice unchanged)
  • Temporary vocal weakness expected first 2-3 weeks (from neck muscle work)
  • Hoarseness, fatigue with talking common during healing
  • Voice returns to baseline by week 4-6

Recovery specifics:

  • Soft food for comfort (swallowing not painful but feels tight)
  • Vocal rest week 1, gentle voice use weeks 2-3
  • Swelling in throat (feels like lump when swallowing) first 2 weeks
  • Visible Adam's apple result immediate though swelling obscures size

Trip Cost Breakdown

Budget Approach (Single-Procedure FMS)

  • FMS surgery (e.g., jaw augmentation alone): $4,500-$6,000
  • Round-trip flights (economy from US/Australia): $1,200-$1,800

Combining FMS with Other FTM Procedures

Many trans men combine FMS with complementary procedures for complete gender affirmation:

FTM Top Surgery (Mastectomy, $4,000-$6,500): Removes breast tissue, reshapes chest for masculine appearance. Can be performed same trip as FMS but typically spaced 3-6 months apart—allows separate recovery periods. If combining same trip, expect extended hospital stay and more challenging recovery.

Hysterectomy ($3,500-$6,000): Removes uterus, sometimes ovaries and fallopian tubes. Usually done laparoscopically. Can combine with FMS trip but requires additional recovery time. Most surgeons recommend spacing 6-12 months from major facial surgery.

Body Masculinization (Liposuction + fat grafting, $3,000-$5,000): Removes fat from hips/thighs (feminine fat distribution), adds to shoulders/chest for masculine V-taper. Can combine with FMS if procedures won't interfere (e.g., can do body contouring + FMS simultaneously, difficult to combine with top surgery due to overlapping surgical sites).

Voice Masculinization Surgery ($5,000-$8,000): Deepens voice through vocal cord procedures. Usually performed separately from FMS—requires vocal rest that conflicts with FMS recovery communication needs.

Considerations for combination procedures:

  • Longer surgery time (8-12 hours total anesthesia)
  • Extended hospital stay (4-5 nights vs 2-3)
  • More complex recovery (multiple areas healing simultaneously)
  • Higher costs but still significant savings vs Western prices
  • Surgeon assessment of safety (not all combinations advisable)

Most surgeons recommend focusing on FMS first if dysphoria primarily facial, then addressing other areas in subsequent trips. Allows full focus on each recovery, reduces anesthesia risk, permits assessment of hormone therapy effects between surgeries.

Common Questions

What does Facial Masculinization Surgery cost in Thailand?

Arrow down

Facial Masculinization Surgery in Thailand costs $8,000-$20,000 for comprehensive packages including multiple procedures. Single procedures (jaw augmentation alone) cost $4,000-$6,000. Comprehensive FMS typically includes jaw augmentation, chin contouring, brow ridge enhancement, rhinoplasty, and Adam's apple creation. Compare to US costs of $20,000-$50,000 for surgery fees alone before anesthesia and facility charges. Thailand pricing includes CT planning, custom implants, surgery, hospital stay, and follow-ups.

Who are the best surgeons for FMS in Thailand?

Arrow down

Dr. Kamol Pansritum at Kamol Cosmetic Hospital is Thailand's leading FMS surgeon with 10,000+ facial gender-affirming procedures since 1997. His team combines plastic surgery, maxillofacial expertise, and craniofacial training—essential for complex bone work required in FMS. Hospital JCI-accredited with dedicated gender-affirming surgical department. Other experienced surgeons practice at same facility, all board-certified by Thai Medical Council and members of professional societies.

Which procedures are included in FMS?

Arrow down

FMS procedures depend on individual facial structure and goals. Common combinations include: jaw augmentation (widening lower face), chin augmentation (lengthening and projecting chin), brow ridge enhancement (creating prominent supraorbital ridge), forehead lengthening (raising hairline), rhinoplasty (widening and straightening nose), cheek contouring (creating angular planes), and Adam's apple enhancement (creating visible thyroid cartilage). Not everyone needs all procedures—surgeon recommends combination during consultation.

How long do I need to stay in Thailand for FMS?

Arrow down

Minimum 21 days, ideally 28 days for comprehensive FMS. This includes 2-3 days arrival and consultation, CT scanning and surgical planning (2-3 days), surgery day, 2-3 nights hospital stay, 14-21 days hotel recovery for follow-up appointments and healing assessment. Single-procedure FMS may allow shorter stay (14-18 days) but surgeon must clear you for travel. Swelling significant at 2 weeks—longer stays allow better healing assessment.

Can I combine FMS with top surgery?

Arrow down

Technically yes, but most surgeons recommend spacing procedures 3-6 months apart. Combined surgery requires 8-12 hours general anesthesia (increased risk), extended hospital stay, and extremely challenging recovery managing both facial swelling and chest healing simultaneously. Most trans men prefer focusing on whichever procedure causes more dysphoria first, then addressing other area after full recovery. If both equally important, surgeon can assess if same-trip combination medically advisable for your specific case.

What if I'm not satisfied with FMS results?

Arrow down

Revision surgery possible but requires minimum 12 months healing before assessment. Swelling takes full year to resolve—perceived asymmetry at 3 months may correct itself by month 12. If genuine asymmetry or insufficient masculinization after full healing, revision can adjust implant size, reposition implants, or add additional augmentation. Revision rates low with experienced surgeons using virtual surgical planning (under 5%). Most dissatisfaction stems from unrealistic expectations or impatience during healing—consultation should establish clear expectations before surgery.

Emergency operation and post-surgery checkup

"I live in New Zealand, and my partner and I went on a holiday in Laos (Asia). I became sick and needed an emergency stomach operation. I reached out to this company, and they have been nothing but helpful.

They helped us with translating in Laos; they organised a taxi driver who was waiting for us at arrival and drove us to the hospital, where they organised a scan for me. I only needed to show my passport and an email when I arrived. I was already in the system for the appointment.

Everything went really smoothly. The company is professional and reliable, and the staff is super friendly too. I would highly recommend this company. I am grateful for the great service this company offered when I was in need."

Konkai Health was always responding fast and helpful if I came up with questions. And the actual surgery had a good outcome also. Now I just ended my last check-up, and I am very delighted how everything went."

Spine Surgery consultation

"The customer care at Konkai Health has been great. I was looking at treatment options for my back. They got back to me fast and gave me options to connect with some top doctors and surgeons in Thailand. The doctors assessed my condition, did a video call, and gave me recommendations to fix it. Thank you. I recommend them for anyone interested in medical tourism to Thailand."

Vasectomy Reversal and Lasik consultation

"I will say you made it so easy to get everything scheduled. I expected it to take much longer trying to find doctors waiting on responses etc.  I was happy with the doctor just going ahead and getting it done on the same day as the first procedure. Everything went well and all the nurses were great. Thanks!"
